The Two Most Important Facts Every Migraine Sufferer Needs To Know

It may seem very simple but it is extremely important to understand that all headaches and migraines are controlled by the Trigeminal Nerves.  These are the paired  V (fifth) cranial nerves.  The Trigeminl nerve is often called the Dentist's nerve because it is the nerve that goes to the teeth and the gums.  In fact, the trigeninal nerve is the single largest input to the brain accounting for over 50% of input to the brain after amplification in the reticular activatng system.  If you have a headache or migraine it is safe to assume that the trigeminal nerve is the innervation.

Nociception (pain input) into the Trigeminal nerve is a primary trigger of all headaches and migraines.  The sixty plus symptoms of TMJ Disorders (TMD) are almost always present either singly or in groups in every patient with chronic headache or migraine.  Changing the input to the trigeminal nerve is the key to controlling, treating and eliminating migraines.  There is excellent evidence that failing to treat the trigeminal nerve associated TMJ problems leads to failure in the treratment of migraine.
IOt has also been shown treating both disorders will lead to successful treatment of migraines and more importantly successful treatment of T%MJ disorders can completely eliminate migraines.

The second fact is that the trigeminal nervous system is closely integrated into the sympathetic and parasympathetic nervous system via the Sphenopalatine (SPG) or Pterygopalatine Ganglion.  It is the largest parasympathetic ganglion of the head and also contains sympathetic and somato sensory nerves.  The Sphenopalatine ganglion is the major control center outside of the trigeminal nerve for controlling headachers.

Every migraine sufferer needs to know these 6 letters "TMJ" and "SP{G"

A ggogle search of these 6ix letters TMJ and SPG will lead to numerous articles showing that the Sphenopalatine Ganglion is the key to eliminating headaches and migraines.  It will also reveal that the SPG Block is the key to eliminating headaches, migraines, cluster headaches and other chronic pain.

Actually, the SPG Block resets the autonomic nervous system and Neuromuscular
treatment of TMJ and MPD disorders eliminates the underlying causes of pain.

An intresting fact in all headache, migraine and
TMJ patients is there has never been found a congenital condition of insufficient medication.  The multitude of medication (very occasionally needed) is the exception not the rule when the SomatonSensory input from the Trigeminal Nervous system is controlled in conjunction with establishing a healthy autonomic nervous system.

Every headache paitient needs to know six letters
TMJ and SPG to improve their quality of life!

OCCIPTAL HEADACHES AND OCCIPITAL NEURALGIA, NOT USUALLY NEUROLOGICAL PROBLEMS

Chronic occipital headaches are usually not a primary neurological phenomenom but are related to postural considerations including breathing, forward head posture, rotation of the head at Occipital-Atlas-Axis joints and referred myofascial pain and nerve compression injury.

These are not problems that should be addressed with medication but rather physical medicine, chiropractic treatment, osteopathic treatment, correction of underlying TMJ and breathing issues.

The connections of the head position to the jaw position has been well explained by the quadrant theorem of Guzay.

Myofascial pain is probably the cause of the majority of occipital pain but is not the root of the problem.  The website www.triggerpoints.net is an excellent primer in myofascial pain conditions.  Nerve entrapment inside of taut muscle bands of myofascial disorders can often contribute to neurologic type symptoms but are actually secondary issues.  Correction of posture and breathing is key.

Treating vs Curing Sleep Apnea: Epigenetic Orthodontics with The DNA Appliance Can Grow a Larger Airway.

There is finally a cur.e for Sleep Apnea after years of Sleep Apnea Treatment.

The DNA Appliance can grow larger healthier airways.

Science now needs to do the research to determine what percentage of patients can be cured and be free of oral appliance and CPAP.

The fields of Sleep Medicine and Dental Sleep Medicine have come a long way since Colin Sullivan first invented CPAP.  Prior to the invention of CPAP it was not uncommon for Sleep Apnea Patients to have tracheotomies to open their airways.

CPAP was a revolutionary change that offered a non-surgical alternative to tracheotomy and was quickly declared the Gold Standard of treatment.

This was a lie!  While CPAP was very successful in terms of efficacy it failed the majority of patients.
Only 25% of patients were actually successful with CPAP and used it on a nightly basis.  The miracle invented by Colin Sullivan failed the majority of patients.

To make CPAP more successful the sleep community encouraged by CPAP manufacturers with very deep pockets declared that CPAP use 4 nights a week for 4 hours a night was successful treatment of sleep apnea.  Never mind that meant efficacy after factored for time used was far below 50%.

This was  good news for the field of Dental Sleep Medicine.  95% of patients offered  choice between CPAP and an Oral Appliance preferred the Appliance but conventional wisdom was oral appliances were less successful than CPAP.

This again was a lie,  Oral Appliance efficacy was equal to or  higher than CPAP after time of treatment was factored in.

It was the determined that CPAP was superior to Oral Appliance was more successful than Oral Appliances for severe sleep apnea.  It was then conceded that for mild to moderate sleep apnea oral appliances were a first line approach to treating obstructive sleep apnea equal to CPAP.  Factoring in efficacy it turns out oral appliances may have higher effectiveness that CPAP even for severe sleep apnea.

It has been reported that Colin Sullivan who invented CPAP wears an Oral Appliance.  What is that all about?  The invention of CPAP was not for mild to moderate sleep apnea but an alternative to tracheotomy for sever sleep apnea.  Colin Sullivan with a milder condition did the same as most patients, when offered a choice he chose the comfortable alternative.

CPAP is actually excellent treatment for the one in four patients who love it and use it all night every night.  For the majority of patients oral appliances are superior to CPAP after factoring in comfort and wear times.

CPAP has greatly improved over the years with features like ramping, smaller quieter light weight machines and a plethora of masks , hoses and features that make CPAP or BiPAP or APAP treatment more comfortable for those who chose to wear it and does nothing for the 65% of patients who abandon CPAP.

Oral Appliances also have issues with fit, comfort, cost, bite changes (due to healing) and problems like being lost, left in motels and eaten by dogs.  Patients prefer Oral Appliances in spite of all these issues but would still prefer not to need an appliance at all.

The DNA Appliance can grow a larger airway and offers a cure for sleep apnea.  The mRNA version can grow the airway and act to hold the airway open during treatment.

There are excellent clinical case studies showing that the DNA Appliance can grow larger airways and cure patients of Sleep Apnea.  The problem is large scale prospective studies are still lacking.

Sleep Apnea is a developmental Disorder that is due to underdeveloped maxillas.  Dr Robert Corruccini has documented this in his book "What Anthropology Teaches us about Orthodontic Diagnosis"  It has been made worse by Retraction orthodontic treatment that is still widely practiced.

It is now possible to cure sleep apnea for some patients with the DNA and mRNA Applianceswhich grow larger airways.

More important, the knowledge now exists to prevent children from devloping sleep apnea as adults.  Early treatment of airway issues can prevent a wide variety of problems and behavioral disorders like ADD, ADHD, Oppositional Disorders, dyslexia and more.

The AAMS (Academy of Applied Myofunctional Sciences)  just had a meeting in Chicago.  Prevention can start at birth by freeing tongue ties to allow the baby to nurse and naturally grow healthier airways.  Dr Kevin Boyd is a pediatric dentist in Chicago who is doing airway treatment in children as young as two years of age.  In England Profesor Mew is growing larger airways with orthoptics and Dr Willian Hang is doing the same in California.

It is time for the public to demand that the orthodontic profession recognize and treat airway in all orthodontic patients, especially children.

It is time for the dental profession to understand that orthodontic referral at 12 or 13 is unacceptable.  It is a dentists responsibility to recognize airway issues and refer fot orthopedic and pneumopedic treatment as early as two years old.

By 7 years old it is too late to prevent ADD and ADHD.  Brain development has already been interrupted in children with sleep disordered breathing.

It is time for medical schools to train doctors thoroughly in early diagnosis of airway issues.

It is time for Oral Myofunctional Therapy be universally recognized for its importance in treating airway and other issue.

John: Yes, doctor. This has been absolutely phenomenal. I’ve got very severe sleep apnea and the thought of wearing a CPAP, I mean, just was a total turn off. I tried it one time and. I mean, it just gagged me and it was just really uncomfortable. So we tried a couple different appliances and the one I’m using right now works just absolutely phenomenal. My wife can get sleep, I can get sleep and it works just so much better than the CPAP machine ever did.

John: Oh, I’d have to think. It’s been at least five years.

Mike: Hi, my name is Mike Olsen and I had been suffering from sleep apnea for many years, and did sleep studies and tried the CPAP on two different occasions unsuccessfully; could not tolerate it. Did more research and found that Dr. Shapira had a dental appliance that might help me with my mild to moderate sleep apnea. Visited him and have been working with that appliance now for roughly six years with tremendous success. Mike: Never. I would never consider going back to CPAP because of the fact that the noise and I’m essentially cured if you will based on my dental appliance.

John originally wore the TAP 1 Appliance by Airway management. The TAP 1 appliance is historically the single most effective appliance for severe sleep apnea. It is very easy for patients to titrate on their own at home. After the snoring is eliminated a follow-up sleep study is required to insure success.Dr Shapira will usually start severe patients with a TAP 1 appliance because of ease of use and extremely successful history with patients.

After several years of wearing the TAP Appliance John changed to a Somnodent appliance from Somnomed. It is a Dorsal Fin Appliance and allows patients to easily open and close their mouth with the appliance in place, drink from glass without a straw and talk easily with the appliance in.

After several years of happily wearing the Somnodent Appliance John felt it was time for a replacement appliance and is now transfering to the Micro 02 appliance which is a modified Dorsal Fin. It is unique due to Cad Cam Manufacture, and that it is the easiest appliance to keek clean. It has a very low profile in the mouth and is extremely comfortable. It is less bulky the the Somnomed appliance and is very well designed for comfort..
